Undertakings by the Responsible Person — payment of fees, leave authorisation, emergency contact, visitor authorisation, collection on termination, joint liability for damage and medical costs.

Emergency Medical Authorisation

Standing authorisation for the Facility to call an ambulance, contact the emergency doctor or homeopath, consent to stabilising treatment, and disclose relevant information to treating practitioners.

Staff-completed risk assessment and admission decision (Admit / Defer / Decline). Submitted after reviewing the Resident's Screening Form. Records room allocation, fee, planned admission date, and updates the Resident profile in Trellis.

Incident · DOC-12

Incident Report Form

General incident report — completed by the staff member who witnessed or attended the incident. Captures persons involved, description, immediate action, injuries, property affected, cross-references and manager review.

Relapse · DOC-13

Relapse Incident Report Form

Specific report for every suspected or confirmed relapse — observations, test conducted, items found, immediate actions, medical outcome, relapse review meeting, decision, updated recovery plan, sign-off and closure.

Monthly Review · DOC-14

Monthly Resident Progress Review Form

Monthly review of wellbeing, sobriety, programme participation, medical and medication, conduct, leave, reintegration progress, recovery plan and resident voice. Counsellor / mentor + Facility Manager review.
